Private School Affidavit

Attention Administrators:  

The California Department of Education would prefer that you file your Private School Affidavit using their web site, http://www.cde.ca.giv/privateschool during the period of October 1 - 15.  If you are starting a new school after October 15, the web site may be unavailable; in this case you may request in writing a blank Private School Affidavit form by sending your request to Attention:  Private School Affidavit, California Department of Education, Policy and Program Coordination Office, 1430 N. Street, Suite 4309, Sacramento, CA 95814.

Please remember that the affidavit is not an application for approval but is a notice of operation, only.

If you are submitting a letter in lieu of the affidavit, you will find a sample letter in the Private School Helps Packet.

FAFSA (Free Application for Federal Student Aid)

This requires some time from the parent, but it should be a high priority!  Normally, you will want to print out the current FAFSA on the Web Worksheet (usually the top link) so you can prepare information that you will need while filling out a FAFSA online.  The worksheet will require a little homework before filling out the application. You will need tax information and possible choices of schools.  Application deadlines vary by state, but are usually in early spring.   Both the student and the parent will have login accounts setup to check the status of the application.  You will also receive information via email.

ACT or SAT College Entrance Exams

Which test should be taken? What are the timelines?  Are there benefits to preparing for college entrance exams?  If so, which method makes the most sense?  A higher score can mean whether the student is offered acceptance, scholarships, and other opportunities.  Even if a student has an impressive high school G.P.A., it doesn't mean he or she will receive a corresponding entrance test score.  Click here to read our ACT/SAT Preparation Information which has helpful information and links.

Employee Fingerprinting

The law requires all employees of organizations that file a letter or affidavit with the department of education to submit finger print cards or live scan information to the Department of Justice. There needs to be a clearance of no criminal record before the staff person begins work.  Many schools choose the Live Scan method instead of the traditional fingerprinting.  Usually the cost of the Live Scan is approximately $75 for a one-time check, but the cost for the notification feature with Live Scan is $99.  The notification feature only costs a little bit more and will notify the ministry/school if a current employee is arrested for a felony related to working with youth.

WORK PERMITS

Students ages 12 through 17 must have a “Permit to Employ and Work” on file with the employer during the term of the employment. This permit is returned to the minor upon termination.

§ 49110.1. Private School pupils; issuance

The Superintendent of any school district may designate the principal or other person having charge of a private school within the district, in which pupils are enrolled pursuant to Section 48222, as a person authorized to issue work permits to pupils of the school, in accordance with this chapter. Where the pupil resides in a portion of the county not under the jurisdiction of the superintendent of any school district, the county superintendent of schools may designate the principal or other person having charge of a private school as the person authorized to issue such work permits. The superintendent of the school district, or county superintendent of schools as the case may be, shall periodically ascertain that the designated person has complied with the requirements of this chapter pertaining to issuing authorities.

IN BRIEF

1. "Almost all minors under the age of 18 are subject to California's child labor protections and must have a work permit in order to work . . . high school graduates under the age of 18, are excluded."

2. "In typical circumstances, after an employer agrees to hire a minor, the minor then obtains from his/her school a brief form (Application) with the title "Statement of Intent to Employ Minor and Request for Work Permit". During the summer, students may obtain a "Summer Work Permit Only--Request For Work Permit."    

3. The Application is completed by the minor and the employer and signed by the minor's parent or guardian." These forms should be returned to the Principal.

4. Some districts require an additional form, Work Permit Guidelines (a document that explains the regulations and must be signed and turned in with the work permit application.

School Medical Records

States may have different health requirement procedures for schools, but most follow the nationally accepted requirements.  A medical record folder in the student's cumulative folder should be maintained.  It will usually contain:

bulleta main record card (usually blue)
bulletentrance health examination document
bullet6th grade hepatitis B vaccine proof (before 7th)
bulletwaivers
bulletcopies of documents used to update the record card (some states allow these to be disposed of once transcribed to the main record card)

Almost all states require a health examination to enter school with proof of childhood vaccines and proof of the  hepatitis B vaccine series before 7th grade. This is used to build the record card. Waiver forms for religious reasons are usually accepted by most states.  Some states allow a waiver for personal reasons.  You may want to use an advanced search engine page, and put the following phrases in the phrase box and enter your state in the additional words box to quickly find your state's forms.  Normally, you want to select a website with yourstate.gov in the web address rather than another school: Health Examination for School, Health Examination Waiver, Vaccine Waiver, School Health Requirements, Cumulative Health Record.   Here are some helpful forms for schools:

Achievement Testing

Standardized achievement testing is a valuable tool administered yearly to evaluate and compare your students’ academic mastery level against students of the same age or grade level.
